Every three years, St. Bernard Hospital conducts a Community Health Needs Assessment (CHNA). The purpose of the CHNA is to identify areas of significant health needs in its service area and to create a strategy to address the top 5 identified needs.
The 2021 CHNA was completed and approved by the Hospital Board of Trustees on November 15, 2021. Please refer to the complete CHNA for the full report. The Implementation strategy was developed by members of the Hospital staff and the CHNA Advisory Committee.
Below are links to the 2021 and 2018 CHNA reports and the 2021 Implementation Plan.
